Quick summary:

You have two ways to do the MCA/YES. You can have the patient sign the physical MCA, or switch to the digital route where a second liveness check serves as the patient's consent through the HCI portal. 

This is based on PhilHealth Advisory PA 2026-0029.




Option 1: Digital, through the HCI portal 

Instead of having the patient sign a physical MCA, the patient does a second liveness check. That liveness check is how they consent to the digital Yakap Empanelment Slip in the HCI portal. You then enter the PCU transaction number into the portal.

One thing to note: the PCU 2 Transaction here in your SMD YAKAP is for your own recording only. We do not submit it to PhilHealth. Submission happens in the HCI portal.

Option 2: Physical signature (the usual way)

Have the patient sign the physical MCA/YES, then submit it to PhilHealth, physically or digitally, depending on how your LHIO wants to receive it. 

Just take note: PhilHealth will be moving MCA submission to the HCI portal app. So if you're doing Option 2, or your LHIO wants you to still submit this way for now, keep this in mind for the transition down the line.

Always check in with your LHIO on how they want you to submit the PCU and MCA/YES.
